How Ozempic dosing works

Ozempic starts at 0.25 mg once a week for 4 weeks, then increases to 0.5 mg. If you need more blood sugar control, the dose can go up to 1 mg and then 2 mg, each after at least 4 weeks on the previous dose. The maximum dose is 2 mg once a week.

Ozempic is approved for type 2 diabetes. For weight management, semaglutide is sold as Wegovy, which uses different dose steps. Wegovy dosage chart and half-life →

How long does Ozempic stay in your system?

Ozempic (semaglutide) has a half-life of about 1 week. The prescribing information says semaglutide stays in the circulation for about 5 weeks after the last dose, as the level halves roughly every 7 days.

Half-life
About 1 week (7 days)
Highest level
1 to 3 days after a jab
Steady state

About 5 weeks after the last dose

The Ozempic label says that with a half-life of about 1 week, semaglutide will be present in the circulation for about 5 weeks after the last dose.

Ozempic is approved for type 2 diabetes. Wegovy contains the same medicine, semaglutide, at higher doses for weight management.

When to increase your Ozempic dose

Each increase comes after at least 4 weeks on the current dose, and only if your blood sugar needs more control. You can stay on 0.5 mg or 1 mg if it controls your blood sugar.

For type 2 diabetes with chronic kidney disease, the US label recommends 1 mg as the maintenance dose.

What is the maximum dose of Ozempic?

The maximum dose of Ozempic is 2 mg once a week in the US, the EU and the UK. The Brazilian label lists 1 mg as the highest recommended dose.

Doses above 2 mg, such as 8 mg, are not approved. See the section below.

Ozempic pens and strengths

Ozempic pens hold several weekly doses. In the US, the 2 mg/3 mL pen gives 0.25 mg or 0.5 mg doses, the 4 mg/3 mL pen gives 1 mg doses and the 8 mg/3 mL pen gives 2 mg doses.

Single-dose prefilled syringes of 0.25 mg, 0.5 mg and 1 mg are also approved in the US.

If you miss a dose of Ozempic

Take a missed dose as soon as possible within 5 days. If more than 5 days have passed, skip it and take the next dose on your usual day.

You can change your injection day as long as the two doses are at least 2 days (48 hours) apart.

Is there an Ozempic 8 mg dose?

No. The largest approved dose of Ozempic is 2 mg a week. The 8 mg printed on one pen is the total amount of semaglutide in that pen, which delivers four 2 mg doses.

Weekly semaglutide doses of 8 mg have only been tested in research studies and are not approved. Never take more than your prescribed dose.

After one dose vs. after months on Ozempic

The level falls by about half every 7 days, whether you took a single jab or have been on Ozempic for months. What changes is the starting point: after several weeks on the same dose the jabs overlap, and the peak is about 2.1 times higher than after one jab, so there is more to clear. As a share of the peak, the timeline in the table above applies either way.

Why Ozempic can feel weaker before your next jab

Ozempic peaks about 28 hours after a jab, once you are on a steady dose. By the day of your next weekly jab, the estimated level is about 59% of that peak, so some people notice more hunger or fewer side effects late in the week. This is expected with once-weekly dosing. If it bothers you, talk to your prescriber rather than changing your schedule on your own.

Can you flush Ozempic out of your system faster?

No. There is no proven way to speed up how your body clears semaglutide. Drinking more water, exercising or fasting does not shorten its half-life, because it is broken down slowly by normal metabolism rather than flushed out. If side effects are the problem, contact your healthcare professional.

Stopping Ozempic before surgery or pregnancy

Pregnancy: the Ozempic label advises stopping at least 2 months before a planned pregnancy because of the long washout period of semaglutide.

Surgery: Ozempic slows stomach emptying, which can matter under anaesthesia or deep sedation. Tell your surgeon and anaesthesia team that you take it so they can tell you whether and when to pause it.

Frequently asked questions about Ozempic

What is the starting dose of Ozempic?

0.25 mg once a week for 4 weeks. It gets your body used to the medicine before the first maintenance dose of 0.5 mg.

How long does it take to reach 2 mg of Ozempic?

At least 12 weeks: 4 weeks on 0.25 mg, then at least 4 weeks each on 0.5 mg and 1 mg, so 2 mg starts in week 13 at the earliest.

What is the maximum dose of Ozempic for weight loss?

Ozempic is approved for type 2 diabetes, and its maximum dose is 2 mg a week. For weight management, the semaglutide brand is Wegovy, which goes up to 2.4 mg and, where approved, 7.2 mg.

Do you need to go up to 2 mg?

No. 0.5 mg, 1 mg and 2 mg are all maintenance doses. Your prescriber increases the dose only if your blood sugar needs it.

How many doses are in an Ozempic pen?

Most US pens hold 4 weekly doses. The 2 mg/3 mL starter pen gives either 4 doses of 0.25 mg followed by 2 doses of 0.5 mg, or 4 doses of 0.5 mg.

Is Ozempic dosing the same as Wegovy?

No. Both are semaglutide, but Wegovy continues to higher doses for weight management (1.7 mg, 2.4 mg and 7.2 mg), while Ozempic stops at 2 mg.

What is the half-life of Ozempic?

About 1 week (roughly 7 days), according to the prescribing information. That is why Ozempic is injected once a week.

How long does Ozempic stay in your system after stopping?

About 5 weeks, according to the label. The estimated level is about 29% of its peak after 2 weeks, about 7% after 4 weeks and under 3% after about 5 weeks.

How long does 0.25 mg of Ozempic stay in your system?

About as long as any other dose, roughly 5 weeks. The half-life does not depend on the dose; a smaller dose simply starts from a lower level.

Why does Ozempic seem to wear off before the next jab?

With weekly jabs, the estimated level before the next dose is about 59% of the peak, so some people notice more appetite late in the week. This dip is expected; talk to your prescriber if it is a problem.

What happens after you stop taking Ozempic?

Semaglutide leaves the body gradually over about 5 weeks, so its effects fade over several weeks rather than stopping at once. Blood sugar and appetite can drift back towards earlier levels, so stop only with your prescriber.

Is Ozempic the same as Wegovy?

Both contain semaglutide from Novo Nordisk, with the same half-life. Ozempic is for type 2 diabetes with doses up to 2 mg, while Wegovy is for weight management with doses up to 2.4 mg and, where approved, 7.2 mg.

How the estimate works

The calculator uses a one-compartment model with first-order absorption, the standard way to describe a weekly injection. It uses average values for semaglutide: a half-life of 7 days, a peak 1.5 days after a single jab and 89% absorption from the injection site. Real levels vary from person to person, and the model does not show how well the medicine works for you.
